Revocation

Revocation is the termination of your privilege to drive. It is required by law upon conviction of certain driving offenses. After your revocation period has ended, your driving privilege will remain revoked until an investigation of your driving record is completed, if applicable, and a new license is issued.

Reinstate Your License

Complete an investigation packet once you are eligible to reapply. Your packet will be reviewed and upon determining eligibility to reinstate, a Permission to Reapply Notice will be mailed to you . Follow the instructions on how to proceed.

For alcohol- and drug-related revocations, you will be required to obtain proof of Future Financial Responsibility, known as an SR-22.

Bring the proof and your notice to any MVD or Authorized Third Party office, fill out an application, pay a $20 reinstatement fee (no personal checks), in addition to an application fee.

If you are required to have an ignition interlock device, Contact MVD before installation to determine if there are any actions on your record that would prohibit your operating a motor vehicle. Proof of installation of a functioning interlock device must be provided prior to reinstatement.

Suspension

Suspension is the temporary removal of your privilege to drive. It is an action taken after a review of your driving record. After your suspension period has ended, your driving privilege will remain suspended until your application for reinstatement is approved.

To reinstate your license

  • contact the court listed on the suspension notice and pay all court penalties due.
  • obtain a court clearance receipt (with a final disposition code and date, violation code and date, and complaint number) or a Court Abstract form.
  • bring the receipt or Abstract to any MVD driver license office and pay a $10 reinstatement fee (no personal checks), in addition to an application fee. If we do not have a digital photo on file, you must apply in person. Otherwise, you may pay these fees by mail or online.

Suspended for Any Other Reason

To reinstate your license, you must pay a $10 reinstatement fee in addition to an application fee. If we do not have a digital photo on file, you must apply in person. Otherwise, you may pay these fees by mail or online at AZMVDNow.gov.

You must obtain proof of future financial responsibility if suspended for one of these reasons:

  • Court-conviction suspension for DUI (If suspended by MVD prior to conviction and it is your first suspension, then future financial responsibility may not be needed.  Contact MVD.)
  • Implied-consent suspension for refusal or failure to complete a drug or alcohol test
  • Violation of vehicle insurance laws

If your reinstatement fee is eligible to be paid online, you will need to:

  • Go to Online Services.
  • In the “Driver’s License and ID” section, select “More Driver Services.”
  • Follow the prompts to create or login to your account.
  • In the “Operator License” section, select “More” then select “Pay Reinstatement Fee.”
  • Follow the prompts and pay with credit or debit card (additional fees will apply) or e-check/bank account number.

If your reinstatement fee is eligible to be paid by mail, you will need to:

  • Print the driver's license reinstatement application. You can type your information into the form and print it, or you can print the application and complete it by hand.
  • Mail or fax the completed application to the address or fax number listed on the bottom of the form with payment:
    • Mail – Payment may be made by check or money order payable to the “State of Michigan” or credit or debit card (additional fees will apply).
    • Fax – Payment may be made by credit or debit card (additional fees will apply).

  • Your driver’s license or ID.
  • Payment by cash, check, money order, or credit or debit card (additional fees will apply).

Schedule an office visit

    • If you had a suspended or restricted driver’s license and the suspension or restriction period is over, you may pay your reinstatement fee through Online Services or by mail. Certain situations will require an office visit to reapply for your license.
    • If you have a revoked or denied driver's license, depending on your driving record, you may have to apply to the Office of Hearings and Administrative Oversight or Circuit Court for an appeal hearing before your driver license can be reinstated.

    • The reinstatement fee is $125. Depending on your situation, you may owe multiple reinstatement fees.
    • Additional fees may be due at the time of reinstatement: Depending on the type of licensing action, you may need to pay for a duplicate license or reapply for a new license. The fees will vary depending on the type of license and whether you have a standard or enhanced license. 
    • Court reinstatement fees: Separate reinstatement fees are due to the court when clearing an FAC/FCJ suspension ($45), or a no proof of insurance suspension or hold ($25). 

  • If your license was revoked, surrendered, or you applied for an ID while your driving privileges were removed, you will need to provide the required identification documents to apply for your license.
